Providence St. Joseph Medical Center is dedicated to providing compassionate, confidential, pre and post-natal care and support for families in our community. We provide care and coordination of services from pregnancy through the first year of the baby's life with the main goal: to keep families together. St. Joe's brings a full suite of services together for each family in need through a single standard of care to improve both short- and long-term health outcomes. Through universal screening and early intervention, we will better connect and support families struggling with substance use and mood disorders to help build healthier families in our community.

Meadowlark Care Coordination
Description
The Meadowlark Initiative® integrates behavioral health screening and services, care coordination, and navigation to community resources into prenatal and postpartum care to keep moms and babies healthy and families together.
All pregnant women and their families need support to have a healthy pregnancy and prepare for their growing family. Our Meadowlark Care Coordinator can help families connect to resources in their community.
